Written by Suzanne McBride:Starting last month, Longmont seniors should have an easier time navigating choices when moving from the hospital to complete their care in a rehabilitation facility or at home, thanks to a new service provided by Stonegate's Central Intake Center (CIC), located in Longmont.

Funded and operated by Stone Gate Senior Living’s Accel at Longmont and Accel at Golden Ridge, the CIC was conceived to streamline the post-acute care referral process for medical professionals, to find the best care option for each client, and to educate patients and families about care choices. The service is free.

CIC admissions coordinators and trained nurses help line up services such as short-term inpatient skilled nursing and rehabilitation (including physical therapy, occupational therapy, and speech therapy,) long-term care, custodial care, and respite care. The system can be accessed by phone or fax, and staffers will work with medical professionals and families to gather clinical and payer information, secure payer authorization, and place the patient. Operators are trained to help callers understand what their health insurance will and will not cover. Additionally, CIC can access ‘bed board’ information for real-time availability of beds.

Since its inception in 2016 as a pilot program in Tyler, TX, the service has grown to include other locations in Texas and Oklahoma and fields approximately 1,000 calls a month. CIC was encouraged by local doctors and hospital administrators to open in Longmont.
